Who is eligible for services? How long are patients eligible for services?
Individuals and families meeting the following requirements are eligible for services:
- Residency in Lane County, Monroe and Harrisburg vicinity.
- Individual/Family GROSS income for the last 3 months is between 85% and 200% of the Federal Poverty Level.
- Not eligible for the Oregon Health Plan (OHP), Medicare, Medicaid, or other health insurance.
- Medical condition is not caused by a motor vehicle accident covered by insurance or a work-related illness or injury that involves Workers' Compensation coverage.
- Not eligible to receive care through any other health care provider.
Patients are screened for eligibility every 6 months from the last screening
